Try a different port, please.
Also, turn off firewall, and check if your port forward works okay.

(By default qBittorrent will try UPnP, but it does not work all the time, some routers are just crap at UPnP.)

You selected the proper proxy type for your proxy?
Are you sure your connection is stable when this happens?

(Many people including myself use proxies and I never seen this "end of file" error.)

You have to forward your ports.
Enable that feature in the PIA client. Their support will help you further.

Is your destination folder writeable?
OS X is more and more restrictive by each version...

It's possible they made "sideloaded apps" even more restricted.

If you run the application from the Terminal, do you see any output?
Do you have anything in the "Execution Log"?
